At this site you can see your account information, change your account portfolio, or even model a loan. The first time you log on, enter your Social Security Number (SSN) as your ID. Use the last four digits of your SSN as your PIN. Once you've entered your account please change the PIN for security purposes. Click on Transactions then Password Change and continue with the change

To Change Pin: Once in your account, click on transactions on the left hand side, click on Password change and continue with the change.

Enrollment Form
Use to enroll in the 401k plan, change employee contribution level, and to designate beneficiaries. When complete, return to your Human Resources Representative.

Loan Request Form
Use to request a loan against your 401k account. Loan minimum of $1000 and maximum of 50% of participants vested balance. Loan can only be issued for the following five reasons: Unreimbursed medical expenses, Purchase of a Primary Residence, Home Improvements, Education Expenses, and Home Foreclosure. When complete, return to your Human Resources Representative.

Distribution Form
Upon termination of employment; use to distribute your account balance either directly to the participant or in the form of a rollover. Please read the Special Tax Notice before turning in your forms. When complete, return to your Human Resources Representative.
